Phase 38: repaired disk-quadrature convergence

This phase holds the corrected longitudinal contour and transverse waveguide shell fixed while refining the polar disk quadrature used in both the ordinary hole projections and the exact sign-split guided self term. The repaired pole is advanced from D12-A32 to D16-A48. A relative decay-rate change below 0.5% with tracked-mode overlap above 0.9999 clears the gate for rebuilding QNM observables.
